Creatine for Women: The 2026 Brain Trial and the AU Pink Tax
1,500mg of creatine HCl per day raised frontal-cortex creatine by 16.4% in perimenopausal women after 8 weeks (p<0.01). That number, from the CONCRET-MENOPA trial[1] published in J Am Nutr Assoc in August 2025, is the first piece of trial-level evidence that a low daily HCl dose can meaningfully push creatine into the female brain. It's also being used to justify pink-packaged AU products that cost up to 4.8x the price of identical unisex monohydrate. We think one of those things is interesting science and the other is a markup you shouldn't pay.
What CONCRET-MENOPA Actually Showed
Korovljev et al.[1] ran a double-blind RCT (n=36, mean age 50.1) on perimenopausal and menopausal women taking 1,500mg/day creatine HCl for 8 weeks against placebo. No resistance training was prescribed. The headline outcomes:
- Frontal-cortex creatine: +16.4% vs +0.9% placebo (p<0.01). Measured by magnetic resonance spectroscopy.
- Reaction time: +6.6% improvement vs +1.2% placebo (p<0.01).
- Mood-swing severity: trended lower, p=0.06.
- Lipid profile: improved, p<0.05.
- No serious adverse events, no weight gain, all interventions well tolerated.
The dose is the interesting part. 1.5g/day is half the bottom of the standard 3-5g monohydrate range that has dominated the literature for two decades. And 5g of HCl only delivers around 3.8g of actual creatine once you subtract the HCl salt weight, so this dose represents roughly 1.1g of pure creatine. It still moved the needle in the brain. That's a genuinely novel finding for the female midlife population.
Why You Shouldn't Extrapolate Yet
CONCRET-MENOPA is one trial of 36 women. There's no monohydrate arm to benchmark against, no replication, and the broader 2024-2025 brain-bioenergetics literature still says the opposite for everyone else. Candow, Forbes, Roschel, and Smith's 2024 review of brain dosing[5] argues that 3-5g/day under-doses the brain because muscle creatine rises by around 20% on standard doses while brain creatine only rises by around 10%. The Smith CABA pilot in mild Alzheimer's[6] used 20g/day for 8 weeks to get global cognition improvements. A depression cohort study showed 10g/day roughly doubled brain creatine gains versus 4g/day.
So one trial says 1.5g HCl is enough for the perimenopausal frontal cortex, and a wall of other trials says the brain needs the high end of dosing to load efficiently. We can't reconcile those positions yet. Our read: CONCRET-MENOPA is a strong hypothesis-generating result, not a protocol you should bet a 12-month supplementation plan on without replication.
If you're already on 3-5g/day monohydrate and tolerating it fine, don't downgrade to 1.5g HCl on the basis of one trial. If you can't tolerate monohydrate at higher doses, low-dose HCl is now a more defensible fallback than it was a year ago.
The Stacy Sims Framework
Dr Stacy Sims[10] is the most-cited clinical voice in the AU female-creatine narrative. Her protocol has stayed stable across appearances on Huberman Lab, Diary of a CEO, Designs for Health, and the Momentous newsletter: 3-5g/day creatine monohydrate, no loading phase, Creapure-grade where possible, taken daily including rest days. Her clinical argument is that women have 70-80% less endogenous creatine storage than men, so the marginal benefit of supplementation is larger.
The Sims protocol predates CONCRET-MENOPA and was built on the broader monohydrate literature[4]. It's the sensible default for general use across the lifespan: muscle, strength, bone geometry, cognition. The 2025 trial doesn't replace it. The two positions sit alongside each other for now, with monohydrate as the gold-standard form and HCl as a niche option for women who can't stomach the standard powder.
Bone Geometry, Not Density
Chilibeck's 2-year RCT[2] (n=237 postmenopausal women, 0.14 g/kg/day creatine plus resistance training) is the other piece of female-specific evidence worth knowing. The result is more specific than the headlines suggest.
Bone mineral density (BMD) didn't change. That's the metric most people associate with osteoporosis risk and the one DEXA scans report. What did change: femoral-neck section modulus and buckling ratio, both geometric markers of bending strength and hip-fracture resistance. The women also gained lean mass and improved their 80m walk time.
The honest framing is that creatine plus resistance training preserves the structural integrity of postmenopausal hip bone without raising density. If a product is selling you creatine on a "bone density" claim, it's overstating Chilibeck. Sell yourself on bone geometry, lean mass, and walking function instead.
The AU Pink Tax, Run as Maths
Now the part that costs you real money. Women now account for 30% of global creatine purchases per Bloomberg's September 2025 reporting[9] on GNC and MULO data, up from 18% in 2020. The market response in Australia has been a wave of pink-packaged, female-coded SKUs at substantial markups over plain unisex powder. The actives are often identical.
We priced six SKUs in the AU market against a unisex baseline (Bulk Nutrients Creatine Monohydrate, $24 per 250g tub, $0.097 per gram of creatine).
- JSHealth Lean+ Powder (women-marketed, 225g, $69.99): $0.467/g. Premium of around 4.8x over the baseline for 5g/serve micronised creatine, bundled with Eriomin and probiotics.
- Buff Bears Creatine (gummy, 5-serve introductory pack, $56): $2.24/g. Premium of around 23x. Bulk packs trim this but still land at 3-5x powder.
- Women's Best Creapure Capsules: around $0.33/g, premium of around 3.4x.
- Naternal Creatine: around $0.30/g, premium of around 3.1x.
- ATP Science Creatine Monohydrate (Creapure): around $0.10/g, roughly flat to baseline.
Run the maths out to an annual supply at Sims' 5g/day: 1.825kg of creatine per year. That's $177 from Bulk Nutrients, $852 from JSHealth Lean+, and an absurd $4,088 from single-pack Buff Bears. The pink-tax gap between Bulk Nutrients and JSHealth on identical-input creatine is around $675 per year.
What You're Actually Paying For
When a women-marketed AU creatine costs 3-5x more per gram than Bulk Nutrients or ATP Science, the markup pays for branding, format (gummies, flavoured powder), and co-ingredients (Eriomin, probiotics, B-vitamins). The creatine itself is the same compound. If you specifically want those add-ins, the JSHealth-style stack is a defensible buy. If you don't, you're paying for the pastel packaging.
The Naternal AU naturopath guide[11] (2026) puts it bluntly: a lot of female-marketed SKUs are "underdosed creatine in pink packaging at a markup." Some women-marketed tubs serve 3g instead of 5g, which lands at parity per serve but below the muscle-saturation dose. The supplement-facts panel matters more than the front of the label.
Gummies are a different category problem. Buff Bears at $2.24/g is fine as a travel format, holiday backup, or a way to get a teenage daughter started on a daily habit. As your everyday creatine source it's around $4,000 per year against $180 for powder. That's a no for us.
HCl vs Monohydrate, Settled in 2025
ISSN's position through 2025[3] hasn't moved: no alternative creatine form, including HCl, outperforms monohydrate for muscle creatine loading. HCl's only validated edge is solubility and GI tolerance. CONCRET-MENOPA used HCl but didn't benchmark against monohydrate, so the 2025 brain result cannot be used to argue HCl is superior. It can only be used to argue that low-dose HCl works at all.
The 2026 default position for the average AU woman is still: micronised creatine monohydrate, 3-5g/day, taken consistently, brand-agnostic. HCl earns its place if monohydrate disagrees with your gut.
Pregnancy, Lactation, and the Menstrual Cycle
No human RCTs of creatine in pregnancy or lactation exist. Guinea-pig and other animal data suggest a protective effect against fetal hypoxia, but LactMed[8] flags potential infant serum-creatinine elevation and notes no transfer data. The default position is to defer to your clinician. Don't self-prescribe creatine through pregnancy or while breastfeeding on the basis of marketing material.
For the cycle, a 2023 Nutrients recovery study[7] (n=39, 20g loading × 5 days) found luteal-phase fatigue index dropped 2.8% versus a 0.2% gain in the follicular phase, and creatine partially rescued that decrement (phase × supplement interaction p=0.048). The clinical takeaway is daily maintenance dosing, not phasic dosing. The cycle modulates your response to creatine; it doesn't change the protocol.
Our Verdicts
Don't pay the pink-tax markup. If the only difference between a pink tub and a unisex tub is branding plus a probiotic, you're $675/year worse off for the colour palette.
Monohydrate remains the gold standard. Sims' 3-5g/day micronised monohydrate is the sensible default across the female lifespan and isn't being displaced by one trial.
CONCRET-MENOPA is genuinely interesting, not protocol-changing. 1,500mg HCl raising perimenopausal frontal-cortex creatine by 16.4% is a real result. n=36, no replication, no monohydrate arm. File it as promising; don't restructure your stack around it.
Don't take creatine in pregnancy or lactation without clinician input. Animal data only. Risk-asymmetry favours waiting.
