Turmeric and Your Liver: The TGA Warning Now on 600+ AU Products
The TGA[1] reviewed 18 reports of liver injury in people taking turmeric or curcumin supplements up to 29 June 2023. Nine had enough detail to link the injury to the supplement, two were severe, and one person died. That is why every one of the 600-plus turmeric and curcumin listed medicines on the Australian register now has to carry a liver-warning label. Our verdict: turmeric on your rice is fine, and curcumin at a sensible dose is a reasonable joint-pain option for most people, but the high-absorption piperine formulations marketed as strongest are the exact ones showing up in the case reports. Match the dose to the job and skip the megadose gimmicks.
The Warning: What Actually Changed
On 15 August 2023 the TGA issued a safety alert[1] about medicines containing turmeric (Curcuma longa and related species) or its active curcumin. The alert was picked up widely, including by the RACGP after the fatal case became public[2]. It led to a rule change: from 1 March 2024, with a 12-month transition ending 1 March 2025, sponsors had to add a warning statement to the label.
The wording tells consumers that in very rare cases these products may harm the liver, and to stop use and see a doctor if they notice yellowing of the skin or eyes, or unusual fatigue, nausea, loss of appetite, abdominal pain, dark urine or itching. It applies to more than 600 listed (AUST L) medicines on the ARTG. The warning does not apply when turmeric is used only as a colouring excipient, and it does not apply to turmeric as a food or cooking spice.
This is the key distinction most coverage buried. Nobody is telling you to throw out your curry powder. The concern is concentrated, medicinal dosing in a capsule, taken daily for months, which delivers far more curcuminoid than any plate of food ever would.
The Case Reports
The clearest Australian evidence is a 2019 case series from the Alfred and Austin hospitals in Melbourne[3]. Two patients, a 52-year-old woman and a 55-year-old man, developed liver injury one month and five months after starting a turmeric supplement. Their liver enzymes normalised after they stopped, and in one case the injury returned when the supplement was reintroduced, which is the textbook signature of a drug- or supplement-caused injury.
The US LiverTox database now classifies turmeric as a well-documented cause of clinically apparent liver injury[4]. The pattern is hepatocellular, with ALT and AST often above 1000 U/L, onset typically one to four months after starting, and most people recovering within a couple of months of stopping. LiverTox also records fatal cases of acute liver failure, consistent with the single Australian death the TGA reported.
Who Is Actually at Risk
The reassuring part is that this is not a dose-dependent poison that harms everyone who takes enough. It is idiosyncratic, meaning it strikes a susceptible minority. LiverTox[4] reports that the HLA-B*35:01 immune-system variant appears in more than 70% of turmeric-associated liver-injury cases, against 10 to 15% of the wider population. That points to an immune-mediated reaction in genetically predisposed people rather than a toxic threshold.
You cannot easily test for that variant before buying a bottle at Chemist Warehouse. So the practical risk markers are the ones you can see. We would be more cautious if you drink alcohol heavily, already take medicines that load the liver, have existing liver disease, or are stacking a high-absorption turmeric product on top of other supplements. And the onset window matters: watch for symptoms across the first one to four months, not just the first week.
The Piperine Problem
Plain curcumin is famously hard to absorb. The supplement industry solved that with piperine, the active compound in black pepper, which raises curcumin blood levels up to roughly twentyfold by slowing how fast the liver clears it. That is the whole pitch behind BioPerine-enhanced products. A typical example on Australian shelves is Fusion Turmeric, which pairs a high-dose Curcumin C3 Complex extract with added BioPerine[7].
Here is the uncomfortable overlap. The TGA[1] stated the risk may be higher for products with enhanced absorption or bioavailability and higher doses. LiverTox[4] goes further, naming piperine-boosted and lipid-nanoparticle turmeric formulations as subsequently linked to several injury cases. So the mechanism that makes a product feel more potent, more curcumin reaching your bloodstream, is also the mechanism that plausibly raises the liver load. Some brands have responded: Blackmores Concentrated Curcumin now markets itself explicitly as having no added black pepper[8].
Does Curcumin Even Work?
This matters because if curcumin did nothing, the answer would be simple: skip it. But it does have real evidence for joint pain. A randomised trial of a Curcugen extract at 1000mg a day cut knee-osteoarthritis pain scores against placebo over eight weeks[5]. A separate head-to-head found curcumin at 1500mg a day produced knee-pain improvements comparable to the anti-inflammatory diclofenac, with fewer gastrointestinal complaints[6].
The important nuance for buyers is that more is not better. Meta-analyses of curcumin for knee osteoarthritis find no clear extra pain relief from high-dose versus lower-dose curcumin. If a standard dose works and a megadose does not add benefit, then paying extra for the strongest, most-absorbed, piperine-loaded formula buys you the higher liver risk without the upside.
The AU Buying Guide
Turmeric is one of the best-selling complementary medicines in Australia, and the range is wide. They are almost all AUST L listed medicines, which means the TGA has been notified and the maker meets manufacturing standards, but the listing does not certify a clinical benefit.

- Match the dose to a real target: roughly 1000 to 1500mg of curcuminoids a day is where the joint-pain evidence sits.
- Prefer a standard extract over a high-bioavailability piperine, phytosome or nanoparticle formula unless you have a specific reason.
- Check whether black pepper or BioPerine is on the label. If it is, weigh whether you actually need the extra absorption.
- If you drink heavily, have liver disease, or take other liver-loading medicines, clear it with your GP first.
- Read the new warning statement and take it seriously in the first one to four months of use.
The Verdict
Turmeric as food is not the problem. The TGA warning is about concentrated medicinal doses taken daily, not the spice in your kitchen. Nobody needs to change how they cook.
If you want curcumin for knee or joint pain, a plain extract at roughly 1000 to 1500mg of curcuminoids a day has real evidence and a lower risk profile than the enhanced-absorption products. Because higher doses do not add benefit, the strongest formula on the shelf is usually the wrong buy.
Skip the piperine megadose products unless you have a specific reason. The absorption boost that makes them feel powerful is the same feature the TGA and LiverTox tie to higher liver risk. If you use one anyway, keep the course short and watch for jaundice, dark urine and unexplained fatigue.
Know when to stop. Any yellowing of the skin or eyes, dark urine, or persistent nausea and right-side abdominal pain in the first few months is a reason to stop the supplement and see a doctor the same week. Recovery is usual once the product is withdrawn, but only if you act on the signal.
